Title: Hot work permit

I am in the process of creating a new hot work permit for the organisation I work for.  As I hate reinventing the wheel can any one be so kind to send me a permit which they use which I can then adjust to meet the need of the organisation.

Once I have this permit completed it will be available for any one else to take and modify for their own use.
