Gas cylinder prosecution

Just got some very sketchy details of a prosecution where a firefighter was killed or seriously injured when a gas cylinder exploded. Anyone got any details?

A lesson to learn for the future if ever advising on decommissioning suppression systems. I've signed up to the HSE weekly newsletter and recommend it as a good way of getting you to sleep at night keeping up with whats going on!
