The job you refer to has become know as the Colville Road (or Collville Road) job, in east London easily 10 years ago.
Following a fire, a number of tunnel construction workers were trapped in a location so remote from the access point that crews had to make up their own BA procedure to reach them as the duration of their sets was not up to the job.
I can't remember all the details, but I recall that numerous dodgy decisions were made and the BA crews at one point needed to almost swim in a sea of silt/slurry to reach the victim(s) Most importantly, all survived as they were very very lucky.
I have no knowledge of the availability of any training material but a comprehensive package was supplied by the LFB to all stations after the incident and it may be possible that some packages stil exist at the back of station libraries or via LFB HQ.
