Oxygen therapy unit fire.

Anyone know of the outcome of the H&SE investigation into the fire in the ICU at Royal United Hospital Bath involving oxygen therapy cylinder?

I've been looking for this as well NT but apart from the link I posted in the 'sponaneous combustion' thread...and this one below.. this is as far as I've got
